Here's one http://www.userbook.com/index.php
quite simple but you can customise it to suit your website.........did one here http://www.userbook.com/gb.php?id=10657 for my local VW club.
I chose this one as it's free and you don't get any adverts or popups! Very easy to access and maintain.

I just use quotes from the actual visitor book in our cottage. It's a good excuse to contact people again to ask if you can use their quote - and sometimes generates another booking .
