Sample feedback form

I need to create a feedback form with some specific questions rather than just asking for general feedback. Does anyone have a sample they could send me that seems to work well for them?

Hi Nemo, we use the following and it seems to work, always filed in and frequent feedback and comments.

Don't want to hijack the thread but my daughter had her first baby on Saturday, delivered by Caesarean section, and was home just a little over 24 hours later. Today she received a " how did we do?" email from the hospital!
So feedback is the new black!
