Catalunya Tourism Tax declaration form

Does anyone know where I can find the tourism tax declaration form? I found once an example on an apartment rental site, but forgot to safe the link.

Looked also on the official site (www.20gencat.cat) but found only information how much you have to pay, and how this new law works.
Have send them an e-mail but still no responds.
